Purpose
The
Home Based Prevention and Intermediate Case sets out the future
strategic direction for the integrated commissioning of voluntary
sector, reablement and clinical support to prevent people needing
an admission to hospital or adult social care service and to
support safe and timely discharges from hospital. It will set out a
programme of transformational commissioning in the context of the
statutory requirements and good practice guidance.

DECISION:
RESOLVED:
1.
That the Home Based Prevention and Intermediate Care
Business Case, as set out in Appendix 1 and exempt Appendix 2 to
the report, be approved. This includes
the decision to proceed on the basis of the Preferred Approach:
Integrated Commissioning of Home Based Intermediate Care and
includes the Council assuming the lead commissioning role for
Service Areas 1, 2, 3 and 5.
2.
Should the Council and/or NHS Cornwall and the Isles
of Scilly Integrated Care Board take the decision not to proceed on
the basis of the recommended approach, the recommendation is to
proceed on the basis of the Council contingency
approach. This includes the Council
assuming the commissioning role for the Council funded services
only, including those in Service Areas 3 (Corserv IDA only) and
5.

That the decision to issue Council led procurements
be delegated to the Strategic Director for Safe and Caring
Communities, in consultation with the Portfolio Holder for Adult
Social Care and Health Partnerships, including any final amendments
to the detail of the procurement approach recommended.
4.
That the decision to award contracts following a
successful procurement process be delegated to the Strategic
Director for Safe and Caring Communities, in consultation with the
Portfolio Holder for Adult Social Care and Health
Partnerships.
