Purpose

This involves working across place to
coordinate transformational priorities as part of Home First and
Urgent and Emergency Care Boards. Project posts support delivery of
the Place Plan such
as liaising with all project leads, reporting and oversight of
Place Plan delivery, for example, Unplanned Emergency Care, Home
First, Mental Health, Frailty, Outpatient Transformation and
Health Inequalities. Securing the above posts creates capacity to
achieve delivery of the refreshed priority areas identified across
place. The project has been raised by commissioning colleagues to
support with system flow and project oversight across Doncaster
Place
