Purpose

Local Authority Designated Officer (LADO)
chaired by XXXXXXX XXXXXXXXX held on the 20th of April with
multidisciplinary stakeholders to discuss the investigation of
several complaints regarding safeguarding concerns raised in
relation to X XXXX XXXXXX XXXX, Director of Heal me Happy
CIC.

XXXXXXXX XXXXXXX XXXXXX XXX XXXXX XXX XXXXXX XXXXXXX XX XXXXXXX
XXXXXX XXXXXXX XX XXXXXX XXXXXX. XXXXXXX XXXXXXXXX XXXX XXXXXXXXX
XXXXXXXX XXX XXXXXXXX XXX XXXXXXXXXXX XX XXXXXXX, XXXXXXX XXXXXXX
XXXXXX XXXXXXXXX XXX XXXXXXXXX XXXX XXXX XX XX XXXXXXX XXXXXXXXXX
XXXXXX XXXXX.
The outcome of the LADO was that the concerns have been
substantiated in their entirety and upon the basis of these
concerns there will be a referral to DBS.

The whole grant agreement is for the financial value of
£XX,XXX and to date Heal me Happy have received
£XXXX.XX.
As the programme manager for Locality Investment, the LADO outcome
and the direct impact is for each stakeholder to decide as to
whether there is a significant impact on the service or activity
XXXX XXXX XX undertaking. I believe the outcome as a direct impact
on the activities XXXX XXXX XX delivering on DMBC behave as part of
the Locality Investment, therefore I am requesting termination
of
the grant agreement with immediate effect as I believe Heal me
Happy have breached the agreement in relation safeguarding
responsibilities.

There may be situations where the grant must be repaid in full by
You, these situations are as follows:
• If in Our opinion, the Project has not been carried
out.
• You have been negligent in relation to Your
safeguarding
responsibilities.
• If it is discovered that You have deliberately misled Us and
have
been dishonest.
• If We have overpaid You by mistake.
• If the payment of the grant is an unlawful subsidy due to
Subsidy
Control.
