Purpose

The overall aim of intermediate care in
Doncaster remains consistent with NICE guidelines for intermediate
care (2017) and ultimately it aims to provide a more streamlined
and accessible, integrated health and social care, whole system
approach to offer intermediate care in the most appropriate setting
to safely meet the needs of individuals. This service promotes the
exploration of options to work together to safely support someone
in their own home environment first before escalating to a bed
based or more intensive service offer.

provision of Intermediate Care via a Section 76 agreement for a
period of 12 months, from 1st April 2022 to 31st March 2023, to
facilitate the
continued delivery of Services provided RDASH.
