Purpose

The single discharge pathway coordinator is a
key senior leadership role to support the local implementation of
the discharge to assess model and the wider home first approach
across Doncaster. Securing the above post creates capacity to
achieve delivery and oversight of the refreshed priority areas
identified across place.
The post holder will assist in reducing deconditioning of older
patients and develop greater consideration of health inequalities.
The project will support system flow and project oversight across
Doncaster Place, the request for this post has been supported by
home first board to deliver key elements of the hospital
discharge
and community support guidance and to ensure that we deliver
national priorities and targets.

To approve funding to appoint a senior single
discharge pathway coordinator to lead the local implementation of
the discharge to assess model and the wider home first approach
across Doncaster (annual contribution of £72,254, band 8a).
The home first board have requested that this post be funded
recurrently via Better Care
Funding (BCF). There has been commitment from both City of
Doncaster Director of Adults, Health, Wellbeing and Culture (DASS)
and the Director of Transformation at the South Yorkshire
Integrated Care Board to ensure that this post can be recurrently
funded. The post previously received sign off from Joint
Commissioning
Operational Group on the 27th June 2023 and received sign off from
Joint Commissioning Management Board on the 1st August 2023. The
post was previously advertised on a short-term temporary basis
which did not attract appointable candidates. Therefore, recurrent
BCF funding has been sourced to fund the post on a permanent basis
that is expected to commence from September 2024.
