Financial arrangements for the provision of
the Dementia Post Diagnostic Pathway via a Section 76 agreement in
partnership with South Yorkshire Integrated Care Board (SYICB) for
a period of
three years, from 1st October 2023 to 31st September 2026.

Contributing services from Rotherham, Doncaster, and South Humber
NHS Foundation Trust (RDaSH) and Age UK Doncaster.

To provide two high quality multi-disciplinary all age dementia
services. One clinical model for pre- and post-diagnostic support
and one community therapy service for people in Doncaster
with dementia, their carers/families, and supporters.

Alternative options considered

For the Council to tender the service for a
separate Dementia contract offer from SYICB’s dementia
support services which was rejected as this would present a
disjointed and duplicate service for Doncaster residents.
