To share work to enhance our capacity and
resilience to support timely and effective hospital discharge
alongside partnership work to prevent social hospital admissions
through community support.

Content

(a)    That Members supported
the ongoing work to reduce hospital discharge delays;

 

(b)    That Members supported
the work being implemented to achieve admission avoidance; and

 

(c)    That Members recognised
the difficulty of the situation facing officers and supported the
ongoing work to reduce hospital discharge.
