Following the completion of a waiver and
conflict of interest form, we would like to order these bespoke
items which meet the terms of the NHS funding to support our
project working in partnership with our NHS colleagues to enhance
the health and wellbeing of residents.

We have been unable to seek three quotations as the item is bespoke
and have struggled to source other potential suppliers that could
complete this order.
