Purpose

As
part of the transformation work in adult social care to create the
conditions to enable people to live well in their communities for
longer, this funding is targeted at working with VCFSE and
Community provision within the local area with a focus on reducing
unnecessary demand at the front door to adult social
care.
 
The
proposal will deliver services that support the following
overarching key project outcomes:
1.
To reduce contacts to Adult Social Care via investment into
community assets that prevent and reduce need and demand that
enable efficiencies that could be reinvested to support
sustainability in the sector.
2.
Build immediate VCFSE capacity to address areas of high demand for
ASC.
3.
Develop closer working relationships in order to have better
connections between the County, District and the VCFSE
provision.
4.
Develop a collaborative and shared approach to early intervention
and prevention, with the benefit of the residents, and targeted
populations, being our primary focus. 5. To support residents in
Lancashire to increase their independence, social inclusion,
connection to their communities and have improved
well-being.
