Purpose

As part of the transformation work in adult
social care to create the conditions to enable people to live well
in their communities for longer, this funding is targeted at
working with VCFSE and Community provision within the local area
with a focus on reducing unnecessary demand at the front door to
adult social care.

Victoria Tomlinson, Senior Commissioning
Manager, on behalf of Clare Mattinson, Director of Strategic &
Integrated Commissioning, took a decision to work collaboratively
with the District Council named to support community development
and reduce inappropriate referrals into adult social care.
