Purpose

The report is about
implementing an Early Intervention approach which is a new way of
working by utilising Occupational Therapists and Social Workers as
the first point of contact for Adult Social Care Services. The
approach will enable them to identify a person’s needs in a
timelier way and provide the most appropriate support to maintain
their independence and support them to remain in their own
home.
