DECISION NOTICE
- House of Flavourz at Unit A, 31 Perry Vale, London SE23 2AR (“The Premises”) has a premises licence PL1078 for the sale by retail of alcohol for consumption on and off the premises between 08:00 to 23:00 hours Monday to Sunday, and the provision of Regulated Entertainment and Recorded Music also between 08:00 to 23:00 hours Monday to Sunday. The premises licence holder (PLH) is Koleishai Reid.
- Angela Mullin-Murrell a Safer Communities Officer - Licensing, for the Licensing Authority submitted an Application to Review the premises licence, processed on 5th June 2026, under section 51 of the Licensing Act 2003, on the grounds of failure to promote the licensing objectives of prevention of crime and disorder; public safety and prevention of public nuisance. The Application included a list of suggested conditions at pages 24-25 of the Agenda.
- This application was served and advertised in accordance with regulation 38 of the Licensing Act 2003 (Premises licences and club premises certificates) Regulations 2005. The last day for receiving representations was 3rd July 2026.
- A written representation was received by PC Sean Rodway from the Metropolitan Police as a responsible authority supporting the premises licence review application. The representation included a series of proposed conditions, set out in full on pages 121-24 of the Agenda, addressing all four of the licensing objectives. The representation was received in time and was deemed relevant as relating to the licensing objectives.
- 27 written representations were received from members of the public in support of the review and a further 21 representations were received from members of the public objecting to the review. The representations were received within the specified time and were considered to be relevant as relating to one or more of the licensing objectives. They were examined by officers and were not considered to be frivolous or vexatious.
- Two video clips were submitted with the application for the licence review by the Applicant and the Licensing Authority received further video footage from the local residents supporting the review. However, since the recordings contained identifiable individuals and personal data, they were not published or circulated as part of the public agenda. They were treated as being confidential and in Part 2 of the Agenda with Members of the Sub-Committee being able to view the video footage. A redacted schedule of the video evidence, including information on the date and time and location of where the footage was taken and an indication of its content but with personal information redacted, was included as pages 152-63 of the public Agenda.
- The Licensing Sub-Committee D held a meeting on the 23rd July 2026 to consider the review application. Safer Communities, Licensing Officer, Kennedy Obazee, introduced the report. The License Review Applicant, Angela Mullin-Murrell, Safer Communities Officer (Licensing Team), made a presentation in support of the review. She concluded saying she was not asking the Sub-committee to revoke or suspend the premises licence but to consider including appropriate conditions to support promotion of the licensing objectives. Ms Mullin-Murrell answered Members’ questions. Three representors who had expressed a wish to speak in support of the review were invited to make their representations and then answered Members’ questions.
- The Chair then invited the premises licence holder, Ms Koleishai Reid to make her presentation against the review. The premises manager, present with her, delivered the presentation and answered Members’ questions. Two Representors who had expressed a wish to speak as objectors to the review were invited to make their representations and they then answered Member’s questions. The review applicant and the speaker on behalf of the premises licence holder, were given equal time (6 minutes) to give their presentations. Each group of representors, both for and against the review, was given an equal time in total (6 minutes).
- The License Review Applicant was invited to sum up their case and given 3 minutes to do so. Finally, the speaker on behalf of the premises licence holder was given 3 minutes to sum up their case.
- In arriving at its decision, the Sub-Committee has considered all the papers contained in the report pack and submitted documents, including written submissions, together with the oral submissions of the applicant and the objector. Also, the viewed videos. The Sub-Committee also considered the Council’s own Statement of Licensing Policy 2026-31 and the latest Home Office Statutory Guidance, revised in February 2026.
- The Committee considered the five possible actions it had the power to take. The decision of the Committee is to allow the current premises licence for the sale by retail of alcohol for consumption on and off the premises and for the provision of Regulated Entertainment and Recorded Music to continue for the hours stated in the licence subject to the additional conditions proposed by the Metropolitan Police and the three of the conditions proposed Licensing Authority. These are set out in ‘Annex A’ to this notice.
- The Committee’s reasons for taking this decision are:
a. The incidents that took place on the 21st and 24th May 2026 were serious failures to achieve the licensing objectives of prevention of crime and disorder, the prevention of public nuisance and public safety. They clearly caused alarm and distress at the time for local residents. Nevertheless, they had happened in close succession and had not been repeated in the following months. The premises management had stated the steps they were taking to prevent any reoccurrence.
b. The set of additional conditions proposed by a Responsible Authority together with those suggested by the Licencing Authority are appropriate and necessary and proportionate to promote the four licensing objectives, in particular to ensure the prevention of crime and disorder.
c. With the venue continuing to operate, the Sub-Committee noted that if a matter arises concerning the premises in connection with any of the four licensing objectives, it was open for any person to instigate a further licence review process.
- There is a right of appeal against this decision. Any appeal should be made to the Magistrates’ court within 21 days beginning with the day on which the Appellant was notified of the decision.
Any person who has given notice of appeal on a licensing decision to the Magistrates Court, or is considering doing so, should not approach council officers for legal advice. A person making an appeal should seek independent legal advice, for instance, from a firm of solicitors or the local Citizens Advice Bureau may be able to assist.

Annex A
General
An incident log shall be kept at the premises, and made available on request to an authorised Local Authority or Police Officer, which will record the following:
a. All crimes reported to Premises License Holder.
b. Any complaints received.
c. Any incidents of disorder.
d. Any refusal of the sale of alcohol.
e. Any visit by a relevant authority or emergency service.
Training
The Premises License Holder and/or Designated Premises Supervisor shall keep up to date policies and staff training records in relation to the following:
Requirements of the Challenge 25 Scheme, drugs, identification and recognition of drunks, identification recognition and responsibilities of dealing with vulnerable persons and the correct procedures to be followed when refusing service regular training must be provided to all staff at least every six months, a record of the training will be maintained for at least 12 months.
CCTV (Prevention of Crime and Disorder)
• The premises shall install and maintain a comprehensive CCTV system. All entry and exit points will be covered enabling frontal identification of every person entering in any light condition. The CCTV system shall continually record whilst the premises is open for licensable activities and during all times when customers remain on the premises. All recordings shall be stored for a minimum period of 31 days with date and time stamping. Recordings shall be made available immediately upon the request of Police or authorised officer throughout the preceding 31 day period. The CCTV should be updated and maintained according to police recommendations.
• A staff member who is conversant with the operation of the CCTV system shall be on the premises at all times when the premises are open to the public. This staff member must be able to show a Police or authorised council officer recent data or footage with the absolute minimum of delay when requested.
• The CCTV must cover all areas of the venue that the public have access to. There should be no obstructions to any internal cameras that creates areas that are not wholly covered by CCTV, apart from within toilet cubicles and directly covering men’s urinals.
Age Verification (Protection of Children from Harm)
• A proof of age scheme, such as Challenge 25, must be operated at the premises where the only acceptable form of identification are (recognised photographic identification cards, such as driving license or passport/Holographical marked PASS scheme identification cards) appropriate signage must be displayed.
• All tills shall automatically prompt staff to ask for age verification identification when presented with an alcohol sale.
• A record shall be kept detailing all refused sales of alcohol. This record should include the date and time of the refused sale and the name of the member of staff who refused the sale. The record shall be available for inspection at the premises by the police or an authorised officer of the Council at all times whilst the premises is open.
• All children under the age of 16 must be accompanied by an adult.
Public Nuisance
• No waste or recyclable materials, including bottles, shall be moved, removed from or placed in outside areas between 23:00 hours and 08:00 hours on the following day.
• Notices shall be prominently displayed at all exits requesting patrons to respect the needs of local residents and businesses and leave the area quietly.
• Loudspeakers shall not be located in the entrance lobby or outside the premises building.
• A noise limiter must be fitted and maintained within the premises. The level of this meter must be set in accordance with required legislation and standards by a qualified sound engineer. The limiter must be sealed in such a way that no unauthorised person can tamper with it. The level should be set in the presence of an authorised officer of the Safer Communities Service of the London Borough of Lewisham. No additional sound generating equipment shall be used on the premises without being routed through the sound limiter device. The device should be calibrated annually and a record kept to be available for inspection by officers.
• During the 30 minute cool down period after sale of alcohol ceases, the volume of music being played must be decreased.
• All windows and doors must be kept shut whilst entertainment is in progress except for the immediate entrance/egress of patrons.
• Patrons permitted to temporarily leave and then re-enter the premises, e.g. to smoke, shall be limited to 4 persons at any one time.
• Patrons permitted to temporarily leave and then re-enter the premises, e.g. to smoke, shall not be allowed to take drinks or glass containers with them.
• Patrons permitted to temporarily leave and then re-enter the premises to smoke shall be restricted to a designated smoking area which does not affect local residents using the pavement to pass the premises.
Supply of Alcohol ON Sales (Prevention of Crime and Disorder)
• There shall be a personal license holder on the premises at all times when the premises are authorised to sell alcohol.
• The sale and supply of alcohol for consumption off the premises shall be restricted to alcohol consumed at the outside tables and chairs.
Supply of Alcohol Off Sales (Prevention of Public Nuisance and Prevention of Crime and Disorder)
• There shall be no sales of alcohol for consumption off the premises after 2200.
Door Supervision
• All staff engaged outside the entrance to the premises, or supervising or controlling queues, shall wear high visibility jackets or vests, due to main road outside.
• A minimum of 2 SIA registered door staff, (where 2, of which 1 must be female) and are all employed by an Accredited Contractor Scheme (ACS) registered company. They must be on duty from 2100 until 30 minutes after closing and when conducting licensable activities or any advertised or private event.
• A register of security personnel employed on the premises shall be maintained in a legible format and made available to police upon reasonable request. The register should be completed by DPS/duty manager/nominated staff member at the commencement of work by each member of security staff and details recorded should include: full name, SIA badge number, time of commencement and completion of duties. The security operative should then sign their name.
Prevention of Crime and Disorder
In the event a serious assault is committed on the premises (or appears to have been committed) the management will immediately ensure that:
a. The police (and, where appropriate, the London Ambulance Service) are called without delay
b. All measures that are reasonably practicable are taken to apprehend any suspects pending the arrival of the police
c. The crime scene is preserved so as to enable a full forensic investigation to be carried out by the police
d. Such other measures are taken (as appropriate) to fully protect the safety of all persons present on the premises.

Event Management Plan
The Premises Licence Holder shall develop, maintain and implement a written Event Management Plan for all promoted events, private functions, live music events, DJ events, themed events and any occasion where attendance is anticipated to exceed normal trading levels. The plan shall include details of event supervision, expected attendance levels, staffing arrangements, security provision, queue management, dispersal procedures, arrangements for dealing with intoxicated persons, emergency procedures and measures taken to minimise disturbance to local residents. The Event Management Plan shall be made available for inspection by authorised officers of the Licensing Authority or Police upon request.
Re-Entry Following Drug Use
Any person who leaves the premises and is reasonably believed by management or security staff to have consumed, possessed or been involved in the use of controlled drugs, including the smoking of cannabis, shall not be permitted to re-enter the premises. Staff shall record any such incident in the premises incident log, detailing the circumstances and action taken.
Dispersal Policy
The Premises Licence Holder shall prepare, maintain and operate a written Dispersal Policy aimed at minimising disturbance to local residents and businesses. The policy shall include procedures for the orderly departure of patrons, active monitoring of the area immediately outside the premises at closing time, arrangements for the management of queues, encouragement of staggered departures where appropriate, and the facilitation of licensed taxi and private hire services. All relevant staff shall be trained in the implementation of the policy and a copy shall be made available for inspection by authorised officers upon request.
