Content

1.         That
condition 2.3 be removed in its entirety and replaced
with:

           
‘That one SIA licensed door supervisor be on duty at the
premises from
21:00 hours on Fridays, Saturdays and Bank Holidays when open until
the premises closes to the public’.

2.         That
there be a designated smoking area at the front of the premises
only.

3.         That the
following conditions be added to the current licence:

(a)        An adequate
CCTV system shall be installed, maintained and operated at the
premises before any licensable activities may take place and
thereafter. The CCTV system shall cover all licensed areas inside
and all entrances and exits. The system shall record continuously
whilst the premises are open to the public and for licensable
activities. Images must be of an evidentiary standard and be date
and time stamped. The system installed must retain all images for a
minimum of 31 days and these must be made available for the Local
Authority, the Licensing Authority or the Police to view or remove
on request. A member of staff trained in working the system and
capable of downloading images shall be on duty whenever the
premises are open to the public.

(b)       An effective
Challenge 25 policy shall be implemented and maintained in place at
the premises. Any persons appearing to be under the age of 25 who
attempts to purchase alcohol must be challenged in respect of their
age and required to provide adequate proof that they are over the
age of 18 before they are sold or supplied with alcohol.

(c)        Each member of
staff involved in alcohol sales will receive adequate training in
the law with regard to alcohol sales and a written record of the
training each person has received shall be maintained. Regular
refresher training will also be given and recorded. The training
records / logs are to be kept on site / electronically or at Head
Office and shall be provided to the Local Authority, the Licensing
Authority or the Police upon request.

(d)       Only photographic
based forms of identification shall be accepted as proof of age in
respect of suspected underage sales. These must be accredited to
the PASS scheme or subsequent equivalent but could also include
passport, HM forces ID card, and driving licence.

(e)        A refusal log
/ form to record all attempts to purchase alcohol that were refused
shall be kept on site and all refused sales recorded. The log shall
be provided to the Local Authority, the Licensing Authority or the
Police upon request.

(f)        An incident
log shall be maintained and kept at the premises, and made
available on request to the Local Authority, the Licensing
Authority or the Police, which will record the
following:

(i)         all
crimes which occur within the venue;

(ii         all
ejections of patrons;

(iii)       any incidents of
disorder;

(iv)       seizures of drugs or
offensive weapons; and

(v)        any refusals
of the sale of alcohol.

(g)        The Premises
Licence Holder shall ensure that all staff working at the premises
shall undertake vulnerable persons / child awareness training, this
training shall incorporate vulnerable person / child exploitation
awareness and what steps to undertake where necessary.

(h)       Off sales of alcohol
shall be in sealed containers from 23:00 hours.

(i)         Except
for exceptional circumstances the Designated Premises Supervisor or
another Personal Licence Holder will be on duty at the premises on
Friday and Saturday evenings and Bank Holidays from 21:00 hours
until the premises close to the public.

 

(j)         Where
the licensee employs / engages any person at or about the premises
in the capacity of a door supervisor, the licensee shall maintain a
register which shall be kept at the premises for a period of at
least twelve months. The register shall note the name, licence
number of any door supervisors employed with the dates and times
they worked and be produced for inspection by the Local Authority,
the Licensing Authority or the Police upon request.

(k)        The premises
shall implement a customer dispersal policy to assist with
minimising noise disturbance to local residents from
customers leaving the premises.  Notices
shall be prominently displayed at all public exits requesting
customers to respect the needs of local residents and leave the
premises and area quietly.

(l)         Doors
and windows are to be closed except for access and egress
after
23:00 hours.

(m)      A dedicated telephone
number for the premises must be given to any resident upon request
to allow complaints to be made directly at all times the premises
are open.

(n)       A complaints log
will be maintained and all complaints from residents will be
recorded therein.  Any complaint logged
will include date, time, name of complainant (if known) and reason
for complaint.  Any action taken in
relation to that complaint to also be recorded.  A copy of the log will be made available to
officers of the the Local Authority, the Licensing Authority or the Police
upon request.
