• Demand in Adult Services is increasing in all areas of
commissioned care and support across the Gwent Region
• We are enabling people with more complex health conditions
to remain at home.
• The level of in-home care and support needed to enable this
to happen is increasing.
• As Individual’s needs become more complex, they move
beyond the legal limits of social care and may be eligible for
continuing health care.
• When an individual is assessed as having a primary health
need then they have entitlement to Continuing Health Care (CHC) and
the Health Board becomes responsible for meeting their ongoing
health and social care needs.
• The proposal is to employ a specialist regional lead social
work practitioner to support the timely identification of CHC
triggers and support the application of CHC eligibility.
