Improving Lives

The Improving Lives programme is a partnership between Coventry City Council, University Hospitals Coventry and Warwickshire NHS Trust, Coventry and Warwickshire Partnership NHS Trust and the Coventry and Warwickshire Integrated Care Board. It was set up in 2023 to address several long-term issues in the city, including high numbers of older people being admitted to hospital, and to reduce the number of people being discharged to care homes.

The programme aimed to:

  • Improve hospital processes to support people when they arrive and reduce the length of their stay.
  • Develop integrated neighbourhood teams with staff from the NHS and social care to support a community-focused model of discharge.
  • Improve intelligence-led oversight through better information sharing between partners.

The meeting was due to be informed that, now that the above elements are in place, the programme would be moving from a change programme to continuous improvement.

Community Integrator

The ICB commissioned UHCW to be the lead provider for the Coventry Community Integrator contract. This contract will build on the approach of the Improving Lives programme, with the goal of realising the ambitions of the Integrated Care System.

The five workstreams within the contract are:

  • Integrated care culture

Population Health Management

The Coventry Care Collaborative has committed to embed a Population Health Management approach to the planning, transformation and delivery of services. A PHM approach seeks to improve the health and well-being outcomes of an entire population, rather than focusing on individuals, by reducing ill-health and addressing the wider determinants of health.

The briefing note for the meeting describes PHM as:

[using] data and intelligence-driven planning and delivery of services to achieve maximum impact. [... including] segmentation, stratification and modelling to identify local ‘at risk’ cohorts. [and] designing and targeting interventions to improve care and support and prevent worsening health for people with ongoing health conditions

Coventry Place Research

This briefing note was prepared to encourage the board to work with and make use of the Coventry Health Determinants Research Collaboration (HDRC), UHCW’s Institute for Health Equity and Social Care, and the Coventry & Warwickshire Integrated Care System (ICS) research infrastructure.

It describes the work of the HDRC, which:

[is] aligned to Coventry as a Marmot City and the One Coventry Plan. [and] Initial priority WDH include Early Years/Best Start, Housing & Homelessness, Local Economy & Employment, Digital Inclusion.
