This meeting of the Health Scrutiny Committee was scheduled to discuss a number of topics related to health and social care in Telford and Wrekin. In particular, there were discussions scheduled about hospital discharge rates and the work of the Telford & Wrekin Integrated Place Partnership.

Hospital Discharge and Intermediate Care

Councillors were scheduled to receive a verbal report about Hospital Discharge rates and Intermediate Care provision.

Telford & Wrekin Integrated Place Partnership (TWIPP)

Councillors were scheduled to receive an update on the work of The Telford & Wrekin Integrated Place Partnership (TWIPP). The report explained that:

TWIPP was established in 2019 under the NHS Long Term Plan to deliver support to residents in Telford and Wrekin with a focus on place-based work to enable further integration of services.

The report identified three priorities for TWIPP in 2024-2026:

• Supporting GP Practices.
• Mental health services & support.
• Healthy Ageing.

The report identified the Joint Strategic Needs Assessment (JSNA), the NHS GP Experience Survey and the Healthwatch Telford and Wrekin GP Access Report as sources that informed the selection of these priorities.

The report explained that TWIPP would focus on community-centred approaches and provided the examples of the Live Well Hubs and the ‘Moving On’ service as examples of the work TWIPP does.

The report noted that TWIPP:

worked in partnership with the ICB, Telford & Wrekin CVS and the voluntary sector as a whole to provide targeted services and support.

Update from the JHOSC

Councillors were scheduled to receive an update on the work of the Joint Health & Overview Scrutiny Committee (JHOSC).

The report explained that the JHOSC had recently:

held an extraordinary meeting in August 2024 that was attended by Senior Officers of the Shrewsbury and Telford Hospital Trust (SaTH) and the Integrated Care Board (ICB) following the publication of the CQC report of hospital performance in May 2024.

The main focus for JHOSC was to review Urgent and Emergency Care provision across Shropshire and Telford and Wrekin.

Update from the Health & Wellbeing Board

Councillors were scheduled to receive an update from the Health & Wellbeing Board.

Work Programme

Councillors were scheduled to review the Committee's Work Programme.

The report identified a number of new items that were suggested for inclusion in the work programme:

• Hospital Discharge and Intermediate Care
• Mental Health
• Review of actions following CQC inspection of Adult Social Care
• Primary Care Access
• Dentistry

The report explained that the work programme was:

delegated to each scrutiny committee by SMB on 3 September 2024.
