The Gloucestershire Health and Wellbeing Partnership convened to discuss several key issues, including the development of neighbourhood health approaches, future meeting schedules, and public questions. The Partnership was also scheduled to confirm and agree the minutes of their meeting held on 29 July 2025¹.

Neighbourhood Health

A presentation on the developing Neighbourhood Health approach within Gloucestershire was scheduled, to be delivered by Emma Crutchlow, GP and Primary Care Network Lead, ICB, and Rosanna James, Executive Director of Improvement and Partnerships, Gloucestershire Health and Care NHS Foundation Trust.

The presentation was expected to cover how neighbourhood health reinforces a new way of working for the NHS, local government, social care and their partners, where integrated working is the norm.

The aim for Neighbourhood Health was described as:

To create healthier communities, helping people of all ages live healthy, active and independent lives for as long as possible while improving their experience of health and social care, and increasing their agency in managing their own care.

The presentation was also scheduled to discuss building on models of care in Gloucestershire, such as proactive frailty models, respiratory care, hypertension, MSK (musculoskeletal) care, diabetes, and dementia care.

The role of vocational rehabilitation² in enhancing health outcomes and employment opportunities was also scheduled to be discussed, including how to support patients in permanent employment to return/remain in work, and providing patients with the tools, skills and adjustments needed to return to work.

Population Health Management³ was also scheduled to be discussed, including the One Gloucestershire linked dataset for health and social care, and Integrated Neighbourhood Teams BI.

The presentation was also scheduled to cover Integrated Locality Partnerships (ILPs), and VCSE involvement in the Neighbourhood Health Model.

Finally, the presentation was scheduled to outline next steps, including strengthening and embedding genuine partnership working across sectors and with communities, workforce development, digital integration, shared care records, and PHM.
